Request an Insurance Certificate

To request an insurance certificate, please complete the form below. Note the following:

• All fields are required.
• If you are not the property owner, an insurance certificate will still be required.
• Provide the full legal business or venue name. If you are unsure of the name, please contact the business directly.

⚠️ Important:

• The insurance certificate will not be valid without the accurate and complete legal business/venue name.
• A $50 processing fee may apply.
• Please allow 7–10 business days for the document to be processed.
• For urgent requests, an expedited processing fee of $100 will apply.

Before a bounce house can be set up at the business/venue:

• Approval from the property owner and an approved insurance certificate are required.
• Some locations require a minimum of 30 days’ notice as well as permits. Please check with the business/venue for specific requirements.

The insurance document will be emailed directly to the business/venue.
